Arizona Department of Gaming Self-Exclusion (casino and/or Event Wagering & Fantasy Sports)
Casino self-exclusion and Event Wagering & Fantasy Sports (EWFS) self-exclusion are separate Arizona Department of Gaming programs—completing one does not enroll you in the other. Choose casino facilities, EWFS, or both. Forms typically require notarization and a photo; in-person/notary assistance is available via the Department.
